Private Family Doctor

UFHH Private Family Doctor is designed to meet the medical needs of your various family members according to their age and physical condition. Each family member will receive an initial medical assessment, based on which a Dynamic Personalized Healthcare Plan is developed. In the months following, UFHH physicians and nurses will pay multiple visits to your home and provide customized medical care, treatment and rehabilitation services.

1) How does UFHH Private Family Doctor work?

Step 1 Initial Medical Assessment

To determine your health condition, the initial medical assessment focuses on the tests, radiology and other assessments considered necessary by UFHH physicians. These can include blood tests, x-rays, ultrasounds, stress tests and so forth. The assessment will integrate medical records and history from other hospitals and physicians.

Step 2 Dynamic Personalized Healthcare Plan

Based on the initial assessment, our physicians will develop a Dynamic Personalized Healthcare Plan that is customized to fit the specific medical situations of each family member. The Plan includes medication, treatment, education, nutrition, monitoring and follow up to ensure optimal healthcare outcomes.

Step 3 Home Medical Visits

According to the Dynamic Personalized Healthcare Plan, UFHH physicians and nurses will conduct follow-up visits in your home and adjust the treatment and health management plan to individual physical condition. When necessary, we will arrange a consultation with UFH specialists and other medical professionals through the green channel. The frequency and number of home visits depends on your family’s medical needs.

Step 4 Monitoring and Management

As part of UFHH’s ongoing Private Family Doctor service, UFHH doctors and nurses will proactively monitor your family’s healthcare progress and medical condition.
